An appointment is a fixed time agreed in advance with a doctor, an officer or a client. In English you make or book an appointment; you never 'take' one.

I'd like to make an appointment.
Bangla says 'অ্যাপয়েন্টমেন্ট নেওয়া', so 'take' comes across. English fixes this verb: you make or book an appointment. 'I'd like' is also softer than 'I want' with a stranger.
What time does the doctor sit?
What are the doctor's hours? / When does he see patients?
'Sit' for holding a chamber is local usage and does not carry into English. The English verb for what a doctor does with patients is 'see'.
Give me a serial for tomorrow.
Could you book me in for tomorrow?
'Serial' in this sense is a Bangladeshi word, and the bare imperative sounds like an order to the receptionist. English asks with a question.
Can I come in Thursday at evening?
Can I come on Thursday evening?
Bangla uses one ending '-এ' for all times. English splits them: on + day, at + clock time, in + month. 'Thursday evening' needs no second preposition at all.
I will cancel my appointment and take another date.
Could I reschedule my appointment to another date?
Cancelling and rebooking are two separate actions; if you only want a different time, the single word for it is 'reschedule'.
