"Doctor, I have gastric." Every pharmacy counter in Bangladesh hears it fifty times a day, and a doctor in Kuala Lumpur or Manchester will not know what you mean — "gastric" is an adjective in English, and on its own it says nothing. The same goes for "my pressure is high" and "my sugar is up". Getting treated starts with naming the thing.

A doctor's visit in English has three stages: describing the symptom, saying how long it has lasted, and understanding the instruction that comes back. The third stage is the one people skip, and it is the one that decides whether the medicine works.

ইংরেজিতে ডাক্তার দেখানোর তিনটে ধাপ: উপসর্গ বর্ণনা করা, কত দিন ধরে চলছে বলা, আর ফিরতি নির্দেশটা বুঝে নেওয়া। তৃতীয় ধাপটাই মানুষ বাদ দেয়, অথচ ওষুধ কাজ করবে কি না সেটা ওই ধাপেই ঠিক হয়।

প্রথমে শব্দগুলো ঠিক করে নিন, কারণ আমাদের চিকিৎসা-ইংরেজির অনেকটাই এই অঞ্চলের নিজস্ব। "I have gastric" → "I have acidity" বা "I get heartburn after meals"। "My pressure is high" → "I have high blood pressure" (সংক্ষেপে "I'm hypertensive")। "My sugar is high" → "I have diabetes" বা "My blood sugar is high"। "Loose motion" → "diarrhoea"। "I have cold-cough" → "I have a cold and a cough"। "I have 102 fever" → "I have a temperature of 102" বা শুধু "I have a high fever"। আর বাংলায় ব্যথা ক্রিয়ার মতো চলে বলে আমরা বলি "My body is paining" — ইংরেজিতে pain বিশেষ্য; বলুন "My body aches", "I have body aches", বা "I ache all over"। সময়ের ব্যাকরণটা এখানে শুধু ব্যাকরণ নয়, তথ্য — ডাক্তার এই সংখ্যাটার উপর ভিত্তি করেই সিদ্ধান্ত নেন। সময়ের দৈর্ঘ্যে for, শুরুর বিন্দুতে since: "I've had a fever for two days." "I've had this pain since Friday." আর ক্রিয়াটা present perfect, কারণ কষ্টটা শুরু হয়েছে অতীতে এবং এখনো চলছে। "I am suffering by fever since two days" — এখানে তিনটে ভুল একসাথে: suffering-এর পরে from বসে, since-এর জায়গায় for, আর tense-টা present perfect হওয়ার কথা। ব্যথার বর্ণনায় ইংরেজি বেশ নির্দিষ্ট, আর ঠিক শব্দটা বললে রোগ ধরা সহজ হয়। জায়গা: "It hurts here" — দেখিয়ে দিন। ধরন: sharp (তীক্ষ্ণ), dull (ভোঁতা), burning (জ্বালাপোড়া), throbbing (দপদপ করা), cramping (মোচড় দেওয়া)। কখন বাড়ে: "It's worse when I bend down." "It gets worse at night." "It hurts when I press on it." এই তিনটে তথ্য — কোথায়, কেমন, কখন — যেকোনো দেশে যেকোনো ডাক্তারের প্রথম তিনটে প্রশ্ন। শেষ ধাপটা সবচেয়ে গুরুত্বপূর্ণ, আর বাংলাদেশে আমরা এখানেই সবচেয়ে বেশি চুপ করে থাকি — না বুঝেও মাথা নেড়ে বেরিয়ে আসি। জিজ্ঞেস করুন: "How many times a day should I take this?" "Before or after food?" "For how many days?" "Are there any side effects I should know about?" "Do I need to come back for a follow-up?" আর সবচেয়ে কাজের অনুরোধ: "Could you write down the dosage for me, please?" এছাড়া একটা বাক্য কখনো ভুলবেন না, বিদেশে তো নয়ই: "I'm allergic to penicillin" — বা যে ওষুধে আপনার সমস্যা হয়, তার নাম। ডাক্তার জিজ্ঞেস না করলেও নিজে থেকে বলুন।

Common Mistakesসাধারণ ভুল

My body is paining.
My body aches. / I have body pain.
'Paining' is a direct translation from Bangla. English speakers say something 'aches', or 'I have pain in…'.
I am suffering by fever since two days.
I've had a fever for two days.
We don't say 'suffering by'; use 'I have a fever'. And with a length of time use 'for' (for two days), not 'since'.
Give me some medicine for headache, no?
Could you give me something for my headache?
The tag 'no?' is a Bangla habit. A polite request ('Could you…?') sounds far more natural to a doctor.
I have pain in my full body.
I ache all over. / My whole body aches.
'Full body' is a literal translation. Natural English is 'all over' or 'my whole body'.

Say when it started and how it feels — 'since yesterday', 'a sharp pain', 'a dull ache'. Details help the doctor. — কবে থেকে শুরু হয়েছে আর কেমন লাগছে বলুন — 'since yesterday', 'a sharp pain', 'a dull ache'। বিস্তারিত বললে ডাক্তারের সুবিধা হয়।
Use polite requests with the doctor: 'Could you…?' and 'Would it be okay if…?' sound respectful. — ডাক্তারের সাথে ভদ্র অনুরোধ ব্যবহার করুন: 'Could you…?' আর 'Would it be okay if…?' শুনতে সম্মানজনক লাগে।
Repeat the instructions back to confirm: 'So, two tablets after meals — is that right?' — নির্দেশনা আবার বলে নিশ্চিত হোন: 'So, two tablets after meals — is that right?'
