A village doctor is the person a village goes to first when somebody falls ill. He is not usually a qualified physician. Most have a diploma, a short course in medicine, or only years of watching and practising, and they are known as village doctors because the village has nobody else. His dispensary stands at the bazaar — a small room with a wooden table, a stethoscope, a torch, a weighing machine and shelves of tablets in strips. He begins in the morning and stays until evening, and on the days the weekly market sits he does not stop at all. He treats what a village mostly suffers from: fever, cough, diarrhoea, worms, stomach pain, cuts and burns. He gives an injection, dresses a wound, and knows which patient must be sent to the upazila health complex at once. He goes to a house at night when he is called, on foot or on a bicycle, which no city doctor would do. He also gives credit, and much of it is never repaid. His limits are real and should be stated. He prescribes antibiotics too easily and often for too few days, which is one reason those medicines are losing their effect across this country. He sometimes treats what he should refer. A confident village doctor with an incomplete training can do harm, and occasionally does. Still, judging him by a city standard misses the point.
